If Box A is checked You must appear in Court
There will be no fine amount filled in on the summons. You must appear at the address on the summons on the Court date and time noted. Failure to appear on the date and time constitutes a separate offense and will result in a warrant being issued for your arrest. After checking in for court and being advised by the Judge, you may then speak to the Prosecuting Attorney. Defendants under 18 years of age must appear with a parent or guardian.

If Box B is checked and You wish to plead guilty
You have the rights as set forth on the back of the summons, including the right to come to court on the date and time indicated on the front of the summons and enter a plea of not guilty. Defendants under 18 years of age must appear with a parent or guardian. If, however, you desire to plead guilty and take care of the matter through the mail, you must sign the Entry of Appearance, Waiver of Rights & Plea of Guilty and mail the Full Payment to the court. Payment must be received in the court clerk’s office no later than 5:00 p.m. three days before the Court date noted on the summons. If Box B is checked and a reduction in points is not shown under the Good Driver Option hearing, your records have been reviewed and it has been determined that you do not qualify for a reduction in points. If you plead guilty by mail to the charge(s) on the face of this notice, you will avoid court costs of $30.

If Box C is checked and you believe you may qualify for the good driver reduction in points
You may receive a letter with further instructions in approximately 10 days. If you do not receive the letter, you must appear in Court on the date and time that is noted on the face of this notice. After checking in for Court and being advised by the Judge, you may then speak to the Prosecuting Attorney.

Defendants under 18 years of age must appear with a parent or guardian.

FINES AND COURT COSTS ARE DUE AND PAYABLE AT THE TIME THEY ARE IMPOSED BY THE COURT
